If you are interested in joining the Utah Association of Healthcare Quality (UAHQ), please download our membership application (pdf 45 kb).

Please sign the application and indicate your form of payment (for individuals making payments through PayPal, you may fax or email your completed application). Membership dues for the state association (UAHQ) are $40 per year due at the first of every year. There are two payment options available to you:

1. Use a credit card or checking account using the secure PayPal checkout process. When using this option, make your payment, and then mail your application to the address below, and indicate you made payment through PayPal. Just click on the Add to Cart button to get started.
   
   
   
2. Make a check payable to UAHQ, attach it to your completed application, and mail it to the address below.
